Insurance information
Please keep in mind, you are responsible for knowing details about your insurance such as your annual deductible amounts and co-pay.
Kindly bring a copy of your insurance card with you to each visit. If your insurance changes, please notify us immediately.
PPO insurers & networks
Our practice lists the insurers and networks below. We may not accept every plan they offer, so please check your specific Illinois PPO plan with us before your visit.

Keep your insurance card handy when you call so we can check the plan name and details.
Medicare
We accept Medicare. Let us know whether you have Original Medicare or a Medicare Advantage plan, along with any supplemental insurance.
Medicare Advantage coverage varies by plan. Call us and your insurer to check whether your doctor is in your plan’s network.
Checking your coverage
- Bring your current insurance card and tell us if your coverage has changed.
- Ask your insurer whether your doctor and visit are covered, and whether you need a referral or approval in advance.
- Ask what you’ll need to pay. Even covered visits can have a copay, deductible, or other costs.
- We offer discounted self-pay rates.

What to bring
- Your photo ID and current insurance card.
- A list of medicines you take.
- Questions you’d like to ask and any changes in your health.
- Test results or records from other doctors that you’d like us to review.
Before and after your visit
- Call or text to make an appointment or change an existing one.
- Ask whether you need to do anything before your visit, such as fasting for a test.
- Before you leave, make sure you know the next steps and when to follow up.
- Call us for help with records, forms, or prescription questions.
