Prepare For CDL Exam
If you use any of the following, bring with you to the exam:
Glasses
Hearing Aids
Bring a list of all your medication
Please inform our Doctor about any and all medical conditions. If you have relevant Medical Records, please have them with you.
During your CDL Exam
Our staff and doctor will review your medical history and perform a physical exam. We will complete the required paperwork for you (Medical Examination Report for Commercial Driver Fitness Determination)
These exams are not covered by your insurance.
